Successful professional recognition in Germany requires not only language skills but also professional qualifications. Targeted preparation for the various exams – from the general language exam through the professional language exam to the knowledge exam – is essential for successful professional integration in the German healthcare system. This section provides a structured overview of the most important steps in exam preparation, outlines different learning and practice options, and explains which state funding options may be available for international physicians, dentists, and pharmacists.
Exams in the recognition process
As part of professional recognition for healthcare professionals, three central exams are generally required. These build on one another and serve to demonstrate both language and professional competence.
General German language exam B2
The general German language exam at B2 level of the Common European Framework of Reference for Languages (CEFR) forms the language foundation for the recognition process and is a prerequisite for further exams in the professional recognition procedure in Germany.
Professional language exam
A passed professional language exam (FSP) certifies subject-specific language competence at C1 level and focuses on communication in the everyday working environment of the relevant profession – such as patient consultations, medical documentation, and exchange with colleagues.
Knowledge exam
The knowledge exam (KP) evaluates professional qualifications and competences in relation to the German training standard and is required when the recognition authority identifies significant differences between the foreign and German training in the recognition certificate.
Alternative: compensatory measure
In addition to the knowledge exam, there is the option of completing a compensatory measure. This serves to specifically address significant difference between the foreign and German training. Participants generally work under the guidance of a qualified professional and supplement missing knowledge through instruction and practical phases.
Preparation options: courses
Thorough preparation is essential for exam success. Specialized courses are available for each profession, specifically designed for the professional language exam and the knowledge exam. These courses last between three and six months, both for the professional language exam and for the knowledge exam.
Preparation courses include practical instruction from experienced teachers, exam simulations, the teaching of specialist vocabulary, and – for the knowledge exam – in-depth coverage of the required theoretical and practical subject knowledge in line with the German training standard.
Preparation options: self-directed learning
For those whose financial or personal circumstances do not allow participation in preparation courses, there is also the option of studying independently at home. Learning materials and strong self-discipline both support this process. Depending on language level and personal situation, self-directed preparation for the professional language exam and knowledge exam can therefore be a worthwhile approach.
Financing and funding
The costs of qualification programs can in certain circumstances be covered by state funding. Various financing options are available for preparation for the professional language exam and knowledge exam, including under certain conditions, the education voucher (Bildungsgutschein) from the Federal Employment Agency or the Jobcenter, as well as the recognition grant from the Federal Ministry of Education and Research. The Federal office for Migration and Refugees (BAMF) issues authorization certificates for professional language exam preparation courses under certain conditions. Further state, federal state-specific, or situation-specific funding options may also be available.
