Doctors: Professional Language Exam – Content

Process and content of the FSP medicine: overview of case history, documentation, patient presentation, assessment, common cases and key preparation guidance.

The professional language exam in medicine is not a conventional language test but is modeled on typical communication situations from everyday medical practice in Germany. The focus is on communicative language ability at C1 level, particularly in listening comprehension, speaking, and writing. Medical expertise is not the primary focus of the exam, but it is required for technically accurate and situationally appropriate communication.

A thorough understanding of the structure, format, and content of the exam therefore forms an important foundation for preparation. This article provides a structured overview of the individual exam sections, typical task formats, and the key requirements of the professional language exam.

Exam structure

The professional language exam generally lasts 60 minutes and is divided into three sections: patient history, documentation, and patient presentation. It is conducted entirely in German and generally takes place at the competent state medical association.

Each exam day accommodates between four and six candidates. The examination board consists of two doctors and a German language teacher, though the exact composition may vary by federal state. The three exam sections are described in detail below.

Section 1: patient history

This section lasts 20 minutes. It may begin with a brief self-introduction from the candidate, for example covering their professional background. This serves as an opening to the conversation and gives the examination board a first impression. A sheet of paper for notes is usually provided. It is advisable to record the key points of the patient history in a structured way. These particularly include the main complaint with accompanying symptoms, previous medical conditions, medication history, surgical history, allergies, family history, lifestyle factors such as smoking, alcohol, and physical activity, as well as the patient’s social and occupational situation. One member of the examination board takes on the role of the patient. The symptoms described are generally based on typical clinical situations. At the same time, the conversation may deviate from the core of the patient history through detailed accounts or side topics. It is therefore important to conduct the conversation in a structured manner, ask targeted follow-up questions, and stay within the allotted time.

The following aspects are assessed in this section:

  • The ability to take a complete patient history, including relevant medical, family, social and lifestyle information
  • The ability to explain possible working diagnoses and appropriate diagnostic and treatment steps in patient-appropriate language
  • Avoiding medical jargon in the conversation with the patient
  • Handling the patient’s follow- up questions, concerns, or uncertainties in an empathetic calm, and confident manner

Taking structured and accurate notes in this section is particularly important, as they form the basis for the subsequent documentation and patient presentation. It should also be noted that the examination board generally does not intervene during the doctor-patient conversation but observes and assesses it.

Section 2: documentation

This section also lasts 20 minutes and follows directly from the patient history taken in the first section. The information gathered in the doctor-patient conversation is documented in writing in the form of a medical letter or structured medical summary.

Accurate medical terminology, clear structure, and grammatical correctness are all essential. The indirect reporting of patient statements – frequently using the subjunctive mood (Konjunktiv I) – is particularly important.

In some federal states, this section also includes writing a referral to the relevant specialist. In many exams, candidates are additionally expected to explain and translate Latin or medical technical terms into everyday German.

Section 3: patient presentation

This final section lasts 20 minutes. The candidate presents the patient case to the examination board, translating the patient history from everyday language into medical terminology and using accurate medical language throughout.

The board may then ask follow-up questions – for example about symptoms, possible working diagnoses, further investigations, or treatment options. The focus is generally not only on highly specialized clinical details but on technically and clearly communicated professional language.

Depending on the federal state, this section may also include an additional communication situation, for example a simulated telephone call between two doctors for a consultation, handover, or referral.

Questions about Latin or medical terms and their translation into German may also arise here.

Exam content

The professional language exam is oriented toward common medical topics and typical situations from everyday clinical practice. The focus is on the ability to assess relevant complaints in a structured way, explain them clearly, and present them using accurate medical language. The following overview shows common case scenarios that are particularly relevant for exam preparation:

  • Cardiovascular conditions: chest pain, shortness of breath, palpitations
  • Respiratory conditions: cough and fever
  • Gastrointestinal conditions: abdominal pain, nausea, diarrhea, and constipation
  • Neurological conditions: headaches, dizziness, loss of consciousness, back pain
  • Endocrine conditions: diabetes and thyroid disorders
  • Urinary tract conditions: flank pain, pain when urinating

This list is not binding for all federal states and other case scenarios may occur. However, the topics listed are among the most frequently recurring examples.

Assessment criteria and results

During the exam, the examination board assesses the candidate’s performance against several criteria:

  • Listening comprehension: accurate understanding of patient statements
  • Speaking: clear, structured, and precise communication with patients and medical colleagues
  • Writing: written documentation that is complete in content, linguistically accurate, and grammatically appropriate
  • Medical terminology: confident and situationally appropriate use of medical terms
  • Patient-centered communication: empathetic, clear, and appropriate communication in the doctor- patient conversation
  • Time-management: the ability to complete all exam sections within the allotted time

The exam result is communicated in different ways depending on the federal state. In some cases, candidates receive the result immediately after the exam. In other federal states, notification is given at a later point – often within a few days to about a week – by post or email.

If the exam is not passed, the main reasons may be explained. Recommendations are often the given, such as completing an observership at a hospital, attending a language course, or undertaking further targeted preparation to improve professional language and communication skills.

There is no fixed nationwide limit on the number of retake attempts. The waiting period before a new exam date can vary, however, depending on the federal state, the state medical association, and the assessment of the examination board. It is often between three and six months.

Practical tips

  • The patient’s full name should be noted down correctly. Errors in personal details can be assessed as serious and may have a negative impact on the exam result. It is therefore advisable to ask for the name to be spelled out and then repeat it back to confirm.
  • If a patient’s statement is not understood, ask specifically for it to be repeated or clarified. Unclear information should not be passed over as it forms the basis for the subsequent documentation and patient presentation. Careful comprehension checking is more important than a formally complete but factually uncertain patient history. Unresolved or misunderstood information can lead to uncertainty later in the exam and negatively affect the assessment.
  • During exam preparation, practice taking notes in key words and using sensible abbreviations. This allows important information to be recorded more quickly and makes it easier to stay within the allotted time.

Understanding the exam as a basis for targeted preparation

Those who are familiar with the structure of the exam and the requirements of each section can prepare in a more targeted and efficient way. The professional language exam is primarily designed to assess communicative language ability in a medical context – medical expertise is not the main focus.

Regular practical simulations and repeated engagement with medical conversation situations in German can help build confidence in the exam format and improve the chances of passing successfully.
