Doctors: Knowledge Exam – Practice

How physicians can best prepare practically for the knowledge exam: from valuable insights into clinical practice with observerships to training with exam protocols.

Those preparing for the knowledge exam (KP) have already completed several important steps on the path to medical recognition in Germany. The KP is one of the last hurdles before the Approbation and should therefore be prepared for early, in a structured way, and with a practical focus. Preparing for the KP differs considerably from preparing for the professional language exam (FSP). While the FSP focuses mainly on language ability in communication with patients and medical colleagues, the KP requires clinical reasoning, medical decision-making, and the confident application of professional knowledge. The suitable training methods differ accordingly. This article presents key methods for training for the knowledge exam effectively and with practical focus.

Practice methods for the knowledge exam

1. FIA Training KP Medicine

The Freiburg International Academy (FIA) offers weekly online training sessions for its course participants, led by specialized doctors. The sessions focus on discussing medical topics, professional exchange with colleagues at the same stage of preparation and consolidating medical terminology.

The two-hour sessions can be a useful complement to self-study. They do not replace independent, continuous preparation for the individual parts of the knowledge exam. Further information on this offer is available on the FIA website.

2. Working with a temporary professional license or completing an observership

Practical work with a temporary professional license continuously strengthens medical language, thinking, speed, and the handling of typical complaints. Theoretical knowledge is directly linked to real treatment situations.

An observership during KP preparation is extremely valuable. It makes it possible to link theoretical content with real patient situations and to better understand clinical decision-making processes. To use this experience for KP preparation, the following points should be kept in mind:

  • Attending morning rounds: They offer insight into clinical decision-making in everyday hospital practice. You can observe the criteria by which doctors formulate working diagnoses, interpret findings, and plan further diagnostic or therapeutic steps.
  • Reading medical reports in a real context: During the observership, pay close attention to how lab values and imaging findings are interpreted in the context of the overall clinical picture.
  • Attending case discussions and tumor boards: Here you can follow how diagnostic findings are assessed, how treatment options are weighed, and how decisions for specific medications, surgical procedures, or other treatment measures are justified.
  • Active thinking: Every case you observe can be used as mental exercise. Consider which diagnosis is most likely, which further examinations would make sense, which treatment could be started, and which risks, contraindications, or special considerations apply.
  • Physical examination: During the observership, practice physical examination on patients where this is permitted. This is how doctors become familiar with the usual examination techniques and procedures in German hospitals. This area is particularly relevant because physical examination can be tested in the knowledge exam.

Important note on observerships: During an observership, patients may only be actively treated or examined if a valid temporary professional license is in place and the facility permits the activity. Those who have only passed the professional language exam but do not hold a temporary professional license can generally only observe everyday clinical work.

3. Simulations

Simulations are a central part of KP preparation, as they train the oral exam conversation realistically. Typical questions from the examination board are practiced, for example on diagnostics, differential diagnoses, treatment decisions, or further clinical procedures.

Simulations can take different forms. Practice sessions with colleagues are easy to arrange, free of charge, and can take place regularly. They help reduce inhibitions about speaking, structure answers more clearly, and learn from the mistakes of others.

Simulations with specialist doctors or experienced teachers can also be a worthwhile addition. They provide professional feedback, correct weaknesses in medical knowledge and communication, and help improve how you present yourself in front of the examination board. Specialized simulation courses, for example at the Freiburg International Academy, can also provide intensive, exam-like preparation.

AI-supported simulations can also be a useful addition to preparation. They allow repeatable case conversations with virtual patients, regardless of location and time. Patient history taking, diagnostic reasoning, treatment planning, and oral expression can be trained in this way. AI models do not replace qualified medical feedback, but they can offer a flexible additional way to practice.

4. Event series: FIA x Dr. House

These sessions are offered by the Freiburg International Academy and are aimed at international doctors with a medical language level of C1. The concept combines fictional case examples with professional discussion: each session is based on an episode of the well-known medical TV series Dr. House, which is then discussed together with specialist doctors.

The focus is on building logical, step-by-step diagnostics thinking. Training includes analyzing symptoms, developing differential diagnoses, planning appropriate examinations, and deriving a suitable treatment plan.

The format offers a varied way to train clinical reasoning in an entertaining and interactive setting. Since the sessions take place online and are free of charge, doctors can take part regardless of where they live and exchange ideas with colleagues. Dates and registration information are published on the FIA website.

5. Working with exam protocols

Working with exam protocols helps you recognize the recurring topics of the respective examination board. At the same time, gaps in knowledge can be closed in a focused way and possible questions can be assessed more realistically.

Once the theoretical part is largely mastered, real protocol questions from previous exams can be included in your preparation.

This type of training gives a clear picture of:

  • The topics and specific questions that recur with the respective examination board, and each examiner’s focus areas.
  • Your own current knowledge gaps that need to be closed.
  • How questions are phrased.
  • Which diagnostics and treatment decisions need to be justified.

Exam protocols and the opportunity to exchange with other doctors from the respective federal states are available in the Community Portal of the Freiburger Bund.

Working through these questions can help you identify recurring focus areas and realistically assess your own level of knowledge. Gaps that are found should then be addressed using your own learning sources. More extensive reference works can also be used for in-depth review. The order is important here: first build a solid theoretical foundation, then work intensively with exam protocols. Without a sufficient medical foundation, there is a risk of memorizing questions without confidently understanding the clinical connections.

Exam success through hybrid learning

Effective training combines theoretical knowledge with the ability to present medical decisions in a structured and convincing way in front of the examination board. The knowledge exam is not passed through memorization alone, but through systematic clinical reasoning, practical application, and growing confidence in the exam conversation.

Preparation should therefore begin early and combine different training methods: analysis-based learning, clinical application, and structured simulations. Every training day helps you apply your knowledge more confidently and get closer to being exam ready.
